LOCUM FAMILY PHYSICIAN – NIAGARA FALLS FHO GROUP + LTC (ST. CATHARINES & VIRGIL)

Dates: Mid-October 2026 to Mid-April 2027

Time Commitment: 3.5 days/week (flexible structure possible if split between locums)

Group Model: Niagara Falls FHO group (just over 20 members)

Coverage Sites: Community practice + 3 Long-Term Care Homes (St. Catharines & Virgil, Niagara)

Summary of Work

We are seeking LOCUM FAMILY PHYSICIAN – NIAGARA FALLS FHO GROUP (22 MDs) + LTC (ST. CATHARINES & VIRGIL)

Dates: Mid-October 2026 to Mid-April 2027

Time Commitment: 3.5 days/week (flexible structure possible if split between locums)

Group Model: Niagara Falls FHO group (just over 20 members)

Coverage Sites: Community practice + 3 Long-Term Care Homes (St. Catharines & Virgil, Niagara)

Summary of Work

We are seeking one or more locum physician(s) to provide coverage for:

• A roster of approximately 1,500 patients (primary care coverage within an established FHO group)
• Long-Term Care responsibilities at three homes:
  • St. Catharines: 30 residents at each of two LTC homes (60 total)
  • Virgil: 80 residents at one LTC home
• LTC time commitment is approximately 1.5 days/week (included in the 3.5 days/week total)

Practice / Environment

• Established, collegial FHO group with strong peer support
• Reliable clinic workflow and continuity for patients
• Option to cover as a single locum or share coverage (e.g., one locum clinic + one locum LTC), depending on fit

Ideal Candidate

• CPSO-licensed Family Physician (or eligible)
• Comfortable with community family medicine and LTC visits
• Collaborative and communicative, with a focus on continuity of care
