Established Downtown Toronto Family Health Team, 6 to 8 week Locum FHO positions available with flexible start date with goal of transition to permanent FHO spot if mutual fit. 6 to 8 week locum is for approximately 1100 patient practice, mix of newborn to geriatrics. Coverage is 6 half-days per week in clinic, including up to 1 evening per week, and 1 or 2 Saturday morning clinics during the locum period. Mix of in-person and phone. Inbox coverage support by dedicated medical assistant. Compensation during locum is fixed split on all FHO income, FFS, access bonus, third party. Permanent FHO position split is 80/20.

If mutual fit is confirmed, the permanent FHO positions would be to either build a practice from scratch or to take over part, or all, of approximately 1100 patients from departing physician, with expectation of ongoing sharing of evening clinic FHO requirements, average of 1 in 6 to 7 Saturday morning clinics, and participation in FHT committee work as needed. The successful candidate will be familiar with Toronto’s health care system and be committed to establishing a long-term practice.

Bridgepoint Family Health Team is a well-established clinic, we have 6 very collegial MD's, an NP, an RN, Social worker, on-site DEP, medical assistants, pharmacist, and sessional specialists. We use Telus PSSuite (and have since the clinic was established over 17 years ago), OCEAN e-referral and messaging with patients. Good TTC accessibility and nearby parking options. We are involved with the East Toronto Family Practice Network and various projects working with our OHT and Primary Care Network.
