Job Description
Position: Family Physician
Organization: Southwest Ontario Aboriginal Health Access Centre (SOAHAC)
Location: Owen Sound Site – #3 - 733 9th Avenue East, Owen Sound ON N4K 3E6
Practice Type / Funding Model: Salary – Primary Services Agreement (PSA)
Hours: Full-time (35 hours/week) – Part-time considered
Start Date: Flexible; coordinated with successful candidate
Compensation: $331,975 annually (full-time PSA rate), prorated for part-time
Benefits: HOOPP (defined benefit pension); extended health and dental benefits
Clinical Setting:
Aboriginal Health Access Centre – interdisciplinary primary care
Population Served:
Indigenous clients and their families; urban First Nations, Métis, and Inuit populations; and non-Indigenous clients accessing culturally safe integrated care services.
Service Mix:
Primary care, health promotion, disease prevention, chronic disease management, acute episodic care.
Interdisciplinary Team:
Traditional Healers, Elders, Nurse Practitioners, Nurses, Mental Health clinicians, Social Workers, administrative support.
Key Responsibilities:
· Conduct assessments, develop care plans, and provide follow-up care.
· Collaborate with interdisciplinary team members within their scopes of practice.
· Provide client-centered care grounded in cultural safety principles.
Qualifications:
· MD with Certification in Family Medicine (CCFP) preferred
· Licence for Independent Practice, or eligibility, with CPSO
· Experience working with Indigenous communities or in CHC/AHAC settings an asset
