Job Description
Position Details:
Ability to perform the following:
· Examine patients, order diagnostic procedures, and consult with other medical practitioners to evaluate patients’ physical and mental health
· Prescribe, administer and monitor medications and treatment
· Provide acute care management
· Vaccinate patients to prevent and treat diseases
· Provide pre-natal and post-natal care
· Advise patients and their families on health care, including health promotion, disease, illness and accident prevention
· Provide counselling and support to patients and their families on a wide range of health and lifestyle issues
· Perform patient advocacy
· Co-ordinate or manage primary patient care
· Provide continuous care to patients
· Supervise home care services
· Report births, death, and contagious and other diseases to government authorities
Qualifications and Skills Required
· Doctor of Medicine (MD) with registration, or eligibility for registration, in good standing with the College of Physicians and Surgeons of Ontario (CPSO)
· Certification, or eligible for certification, by the College of Family Physicians of Canada (CFPC)
· Current CMPA membership in good standing (or eligibility for)
· Experience with chronic disease management, mental health and marginalized populations considered an asset
· Demonstrated commitment to community-based primary care, health equity and patient-centred practice
· Strong communication, clinical reasoning, organization skills and working as a team member
