JOB/ORGANIZATION INFORMATION

The Tilbury District Family Health Team is seeking a Primary Care Locum to join our multidisciplinary team and provide high-quality care to our rostered patients.

Location: Tilbury, ON

Position Type: Locum – October 1, 2025-December 31, 2025

Start Date: October 1, 2025

Chatham-Kent has approximately 60 family physicians working in Chatham-Kent with 7 working at TDFHT. The Interdisciplinary team at TDFHT consists of the following: NPs, RNs, RPNs, Social Workers, Pharmacist, Chiropodist, Phlebotomist, Dietitian, Respiratory Therapist, and Kinesiologist, as well as administrative staff. The EMR is Accuro.

Qualifications:

• Licensed and in good standing with the College of Physicians and Surgeons of Ontario (CPSO)
• CMPA or equivalent liability coverage
• Experience in a primary care or family health team setting is preferred
• Proficient with EMRs and virtual care platforms

Responsibilities:

• Provide primary care to rostered and same-day patients
• Collaborate with interprofessional team members (e.g., Physician’s,  Nurse Practitioners, nurses, social work, Pharmacist, Respiratory Therapist, Kinesiologist and administrative staff)
• Maintain accurate and timely EMR documentation
