Job Description
We anticipate having one vacancy for a family physician in 2024.
The City of Lakes Family Health Team is an interdisciplinary team of local family physicians and health care professionals working collaboratively to provide comprehensive primary health care services to their patients. Our team includes family physicians, nurse practitioners, registered nurses, a pharmacist, a dietician and a social worker. We are looking for an interested family physician who wants to practice in a collaborative team environment. All patients are rostered to one of our family physicians. Individual physicians set up their practice at one of our clinic locations.
Physicians on our team are members of a Family Health Network (FHN). Based on an agreement with the Ontario Ministry of Health and Long-term Care, the FHN uses a blended payment model that includes a capitation payment derived from the size of their individual patient roster. Other special premiums are paid to the physician for several areas of care including preventative care, hospital services, palliative care, home visits, obstetrics and prenatal care. All patients are rostered to an individual physician. Physicians and nurse practitioners have a rotating schedule for after hour’s clinics provided to our patients by appointment only. Physicians can refer their patients to any member of our interdisciplinary team.
Our clinics are networked using electronic medical records instead of traditional paper charts. These records are stored and protected in our computer system. Ultimately, this means better communication between all members of the Family Health Team. It will also allow your medical records to be easily accessible, and securely shared with specialists, hospitals and other health care providers as needed. No matter where you are or whom you are seeing, that health care provider will have up-to-date information concerning your medical history, current medications and recent test results. A state-of-the-art broadband data network connects our clinics. We are a member of the Ontario Telemedicine Network (OTN). Telemedicine is the delivery of health-related services and information using telecommunication technologies.
We are committed to seamless care for our patients. That means that all doctors on our team have hospital privileges; therefore, they can visit you if you have been admitted to Health Sciences North. Our clinical team provides clinical placements for medical students, family medicine residents, nurse practitioner students and other learners from health-related disciplines. We believe that learners who experience clinical opportunities in an interdisciplinary setting are more likely to join practices such as our family health team once they have graduated.
