CORE RESPONSIBILITIES OF THE POSITION
The primary focus of the Physician is to conduct assessments and work with the inter-professional team to provide care and treatment of health conditions to assist people living with chronic illness and social difficulties. As a key member of the interprofessional team supporting the Leamington programs, the Physician will:
• Perform assessment, diagnosis and treatment, health education, counselling and other clinical activities according to the College of Physicians and Surgeons of Ontario (CPSO) standards of practice
• Work collaboratively with other health care providers to provide optimal care for the Centre’s clients, including active collaboration with Nurse Practitioners
• Provide care and treatment of health conditions by supportive, preventative, therapeutic and rehabilitative means in order to attain/maintain optimal function of the client
• Provide clinical assessments addressing the physical and emotional needs of clients, including those with multiple co-morbid conditions and chronic pain evaluation and management
• Contribute to chart documentation, participate in chart reviews and case conferences and make referrals
• Implement appropriate strategies by working with the inter-professional team using a client-centered approach
• Actively participate in the teaching/learning process with clients, families, students and team members about best practice and evidence-based care
• Perform other duties that support the mission and vision of the organization and/or as assigned based on position responsibilities and/or site requirements.
SKILLS, KNOWLEDGE & EXPERIENCE REQUIRED
• Minimum of one (1) year experience working as a primary care Physician
• Experience in a community health setting or combination of community health and hospital or public health settings
• Strong attention to patient and family-centered care and establishing therapeutic relationships required
• Experience in program and service development, implementation, monitoring and evaluation required
• Demonstrated ability to work in an inter-professional team environment required
• Strong communication and excellent interpersonal skills, combined with good analytical, problem solving, organizational and administrative skills
• Evidence of on-going professional development required
• Clean driver’s abstract, current, valid Class “G” Driver’s License and reliable transportation to support travel across Windsor-Essex County and the province, as required
• Bilingual (English/Spanish) language skills an asset with strong ability to speak and communicate fluently in both English and Spanish.
• Bilingual (English/French) language skills an asset.
EDUCATION AND CERTIFICATE REQUIREMENTS
• Medical degree from a recognized University
• Member in good standing, or eligible for registration with the College of Physicians and Surgeons of Ontario
• General license to practice from the CPSO
• Certified Family Physician Certification (CFPC) or equivalent and practical experiences an asset.
