Job Description
Community Care City of Kawartha Lakes (CCCKL) is an innovative, multi-service community agency offering a range of community support and clinical services. We are seeking a dedicated and compassionate Part Time Physician to join our healthcare team. The successful candidate will provide high-quality, patient-centered care in a collaborative and supportive environment.
The Physician provides comprehensive primary care within a community health setting, working collaboratively with an interdisciplinary team to deliver high-quality, patient-centered care. This role focuses on health promotion, disease prevention, treatment, and community outreach to support the holistic well-being of individuals and families.
Key Accountabilities:
· Provide comprehensive medical care including assessment, diagnosis, and management of acute and chronic health conditions.
· Deliver same-day and walk-in services for clients with acute or urgent care needs.
· Conduct health assessments, order and interpret diagnostic tests, prescribe medications, and provide follow-up care.
· Perform minor procedures such as wound care, suturing, and immunizations.
· Collaborate closely with nurse practitioners, nurses, social workers, dietitians, and other health professionals to ensure coordinated, seamless care.
· Participate in case reviews, clinical team meetings, and quality improvement initiatives.
· Provide consultation and mentorship to clinical staff and learners as required.
· Maintain accurate, timely, and confidential medical documentation in accordance with professional and organizational standards.
· Engage in health promotion and education initiatives, including community outreach and patient self-management support.
· Contribute to the development, delivery, and evaluation of clinical programs such as prenatal care, chronic disease prevention, cardiovascular health, and palliative care.
· Support “seamless care” by ensuring appropriate referrals and coordination among specialists, hospitals, and community partners.
· Be accessible during scheduled hours and provide on-call coverage as required.

Requirements:
· Doctor of Medicine (MD) and registration (or eligibility for registration) with the College of Physicians and Surgeons of Ontario (CPSO).
· Certification in Family Medicine from the College of Family Physicians of Canada (CFPC) preferred.
· Current CMPA membership in good standing.
· Demonstrated commitment to community-based primary care and interdisciplinary collaboration.
· Excellent communication, problem-solving, and organizational skills.
· Commitment to health equity, cultural safety, and patient-centered practice.
· Experience in community health, chronic disease management, or health promotion considered an asset.
Compensation:
The wage for this position, provided in accordance with Ontario Health (East) funding for this role, is $170.24 per hour.
