Take Over an Established FHO-FHT Practice in a Beautifully Renovated Clinic

A physician in a well run FHO/FHT practice on the Hamilton Mountain is looking for a successor to take over the practice on/after June 2027. As part of a 24 member FHO and the Hamilton Family Health Team, this is an exciting opportunity for anyone looking to take over a well supported practice offering team-based care in a beautiful newly renovated clinic space.

As part of the Hamilton Family Health Team, this practice receives the support of a variety of allied health professionals including a full-time RN, a clinical pharmacist (1 day per week), a dietitian (1 day per week), a psychiatrist and mental health RN (on-site 2 times per month), and a full-time mental health counsellor. The practice is also conveniently co-located with 2 FHO colleagues as well as a third physician who is not part of the same FHO.

The successful candidate will be caring for a roster of approximately 1,200 well-managed patients and the practice currently sees an average of 15 – 20 patients per day. Current practice hours are Monday, Tuesday, Thursday, and Friday from 9 am to 4 pm plus Wednesday from 9 am to noon. As part of a large 24 member FHO, the practice provides after-hours weekday coverage to the FHO on a 1 in 24 rotation as well as weekend on-call coverage approximately 2 weekends per year (with draws to determine who will cover holidays each year).

The practice uses Accuro EMR and is supported by an experienced administrative staff who handles all office operations as well as a booking clerk. The newly renovated location offers a bright and modern space with free parking, 3 exam rooms, a private office, a shared kitchenette, a nursing station, a reception area/waiting room, and separate staff and patient washrooms. Located near the scenic Hamilton Mountain brow within walking distance to the Juravinski Hospital and is a short drive away from both the Lincoln Alexander Parkway and downtown Hamilton. The building also holds a pharmacy, Pain clinic, Infectious disease - Internal Medicine clinic, and a physiotherapy clinic is located 2 doors down.
