An established and well-run FHO-FHT practice in south Hamilton Mountain area is seeking a family physician to take over the practice. This is an excellent opportunity for those looking for a well-supported practice in a rural/urban setting just minutes away from a larger urban centre.

This solo practice is part of a large 26-physician FHO and the Hamilton Family Health Team (HFHT). With a roster of approximately 1,500 patients, the practice typically sees between 20 – 25 patients per day through in-person visits.

The practice receives full administrative support from 2 Medical Office Assistants, and as part of the HFHT the practice also receives the support of a mental health counsellor, a registered dietitian and a part-time nurse practitioner.

The clinic space provides 4 exam rooms, an office, wheelchair accessible washroom, waiting room and free parking.

Patient records are managed electronically through OSCAR EMR, and the practice also uses OCEAN for e-referrals and to securely communicate with patients.
