Job Description
The Mount Sinai Academic Family Health Team is excited to announce the creation of three new Family Health Organization (FHO) positions at our modern, team-based primary care clinic in Vaughan. We are seeking Family Physicians to establish new, full-scope community practices within our supportive interdisciplinary setting. This is a unique opportunity to build your own patient roster while contributing to a vibrant academic environment committed to excellence in patient care, education, and system improvement.
About the Clinic
Conveniently located near Bathurst Street and Rutherford Road, our large, bright, and fully equipped clinic includes 28 exam rooms and is home to a collegial group of nine family physicians. Designed to support both provider well-being and high-quality patient care, the clinic offers:
- Dedicated physician workspaces
- Electronic medical records (TELUS PS Suite and Ocean)
- Optional use of a standalone AI scribe tool
- Interdisciplinary team: RN, NP, social worker, dietitian
- Turnkey operations including IT support, clinical and administrative staffing, supplies, and EMR support
Physicians at the Vaughan site work within a blended capitation model (FHO), contributing a fixed percentage of their earnings toward shared overhead. For eligible new physicians, the Ministry of Health’s Income Stabilization Program offers guaranteed compensation up to 12 months, providing financial stability while building a patient roster.
Sinai Health manages all administrative and operational aspects of the clinic, allowing physicians to focus on patient care. Services include appointment scheduling, patient rooming, vital signs, specimen collection and handling, billing support, and patient communications—ensuring an efficient and well-supported clinical environment.
As an academic Family Health Team embedded within a hospital system, the clinic benefits from access to hospital-level infrastructure, integrated health system supports, and ongoing professional development opportunities. This unique setting enhances the experience for both providers and patients, while offering meaningful opportunities to engage in teaching, research, and quality improvement.
Practice Details

· Six half-day clinics per week, including one evening clinic (5–8 PM)
· Participation in Saturday urgent care clinic (1 in 10-12 rotation)
· Optional hospital, home visit, or obstetrical responsibilities available, depending on physician interest
Academic Expectations
Physicians are expected to teach medical students and residents from the University of Toronto’s Department of Family & Community Medicine. Additional academic opportunities include participation in curriculum development, quality improvement projects, leadership and primary care research and scholarship initiatives.

- Certification in Family Medicine (CFPC)
- Eligible for licensure with the College of Physicians and Surgeons of Ontario
- Eligible to work under a Ministry of Health FHO agreement
- Committed to comprehensive, team-based care in an academic environment
