This clinic is a physician-lead initiative that has been serving the medically and socially most complex patients in the Western Communities since 2019. It is a multidisciplinary organization including FPs, NP, SW, RN, Pharmacy and Psych Consult Liaison. In collaboration with the Victoria Primary Care Network (PCN), Ministry of Health, Island Health and Divisions of Family Practice the HCCT is now expanding its successful model into Victoria. We are looking for another Family Physician interested in innovative primary care to join our team.
Key Points:
Meaningful work with high-needs, under-served population
Direct patient care, charting, admin and lab time compensated at competitive APP rate
Mix of in-office, home visits and work from home shifts available
Option of flexible, parent-friendly scheduling e.g., 9:30am-2pm shifts if desired. Also compatible with ER/Hospitalist schedules
Effective team-based care with a shared panel and experienced NP/RN/SW colleagues
Supportive, fun and collaborative environment
Daily team meetings for sharing cases
Collaborative relationships with Island Health,
Victoria Mental Health and other Community partners
Clinical mentorship from award-winning colleagues, including in psychotherapy skills
What we are looking for:
Dedicated, compassionate and collaborative physicians
Ability to work as a team with Nursing, NP and Allied health in a non-hierarchical manner
Open to applicants in all career stages, from new grad to semi-retirement
Open to physicians currently practicing as FPs, ERPs or Hospitalists
We value quality of care over speed/efficiency (but some of both is nice)
Shared philosophy of trauma-informed, culturally-safe care essential
Either interest in or experience with mental health and addictions
