Job Description
Sherbourne Health is a community-based health facility providing innovative primary health care to all the diverse populations of southeast Toronto. Our services have a special focus on people who are underhoused, the 2SLGBTQ community, and newcomers to Canada.
We are currently seeking a locum physician for our Urban Health Team. This would be a 1-year term to cover a leave of absence. The preferred start date is August 2025. The position is 3 days per week, hours are regularly 9am-5pm with one evening clinic every 2-3 weeks and Saturday morning clinics once every 2-3 months. There is one week of after-hours phone call coverage also every 2-3 months.
The physician will work with a dynamic clinical team to provide primary care to our priority populations including people experiencing homelessness/are underhoused, individuals with addictions and mental health issues, newcomers to Canada and the 2SLGBTQ population. Care is a mix of prebooked appointments and unscheduled (drop-in) access. Sherbourne’s medical services are delivered in a Family Health Team model, with physicians under an Alternate Payment Plan with a guaranteed competitive hourly rate. Benefits include a well-supported interdisciplinary team (nurses, nurse practitioners, mental health counselors, dieticians, client resource workers, lab technicians, diabetes education team, onsite pharmacy), well equipped offices, full EMR (Accuro), EMR/IT support, UptoDate access, HRM, EMR integrated eConsult, Ocean e-referral, and OLIS. Coverage for time off is provided by the group through shared care and regular locum availability. Teaching opportunities are available as the clinic regularly hosts learners as a University of Toronto teaching site. Sherbourne Health was accredited with exemplary standing in 2023 by Accreditation Canada and in 2017 won the OCFP’s Family Practice of the Year.
The successful candidate will work collaboratively within an interdisciplinary care model, under an anti-oppression framework. Candidates must be motivated and comfortable working with highly complex clients. Experience with Newcomer health, 2SLGBTQ health including transgender care, HIV primary care, mental health, substance use disorders and an appreciation for the social determinants of health is beneficial. Training in transgender care through Rainbow Health Ontario is offered as part of our onboarding.
