Concord Family Health Center is currently seeking a Family Physician to join our growing primary care team. We are looking for a qualified and motivated physician who is interested in providing comprehensive family medicine services to patients of all ages in a supportive outpatient clinic environment.

The successful candidate will work alongside experienced physicians, administrative staff, and allied health professionals in a modern family practice setting focused on accessible, patient-centered care, preventative medicine, chronic disease management, and continuity of care.

Key Responsibilities

The Family Physician will be responsible for assessing, diagnosing, treating, and managing patients with a broad range of primary care needs. Duties may include performing comprehensive medical assessments, providing routine checkups, managing acute and chronic conditions, ordering and interpreting diagnostic investigations, prescribing medications where clinically appropriate, providing preventative care, and coordinating referrals to specialists and community services.

The physician may assess and manage patients with common medical concerns, chronic diseases, minor injuries, mental health concerns, women’s health needs, pediatric and geriatric care needs, preventative screening requirements, and other family medicine concerns.

The physician will also be expected to maintain accurate medical records, communicate effectively with patients and other healthcare providers, support roster growth where applicable, and contribute to a professional, patient-centered clinical environment.

Qualifications

Applicants must be eligible for or currently hold registration with the College of Physicians and Surgeons of Ontario (CPSO). Candidates should have training and experience in Family Medicine, with an interest in comprehensive primary care, preventative medicine, chronic disease management, and community-based healthcare considered an asset.

Strong communication skills, professionalism, and the ability to work collaboratively within a multidisciplinary clinic setting are required.

Compensation and Schedule

This is a full-time position based on 40 hours per week, with compensation at $125.00 per hour under a fee-for-service payment model. Scheduling may be flexible depending on clinic needs and physician availability.
