ABOUT THE OPPORTUNITY

NL Health Services – Eastern Rural Zone invites applications for a permanent full-time Family Care Team Physician at Carbonear General Hospital in Carbonear, NL.

The successful candidate will join an interdisciplinary Family Care Team delivering comprehensive, coordinated, patient-centred primary health care to the Carbonear and Rural Avalon community. The Family Care Team model brings together physicians, nurses, nurse practitioners, and other allied health professionals to provide accessible, team-based care across a broad range of primary care services.

The physician will provide comprehensive primary care services including chronic disease management, preventive care, acute care, and coordination with specialist and community services. This is an excellent opportunity for a physician seeking a collaborative, team-based practice environment in a supportive and well-resourced community setting.

Carbonear General Hospital serves approximately 50,000 people in the Rural Avalon Region and provides a broad range of acute and community health services. The hospital has 70 acute care beds with a 6-bed ICU and a full range of specialty and support services.

NL Health Services is affiliated with the Faculty of Medicine at Memorial University of Newfoundland and Labrador, and academic interests are encouraged and supported.

QUALIFICATIONS

• Medical Degree (MD) from a recognized institution
• Eligible for licensure as a Family Physician with the College of Physicians and Surgeons of Newfoundland and Labrador (www.cpsnl.ca)

• Must satisfy credentialing requirements of NL Health Services including a satisfactory criminal record check and vulnerable sector check
• Commitment to team-based, patient-centred primary care
• Experience or interest in chronic disease management considered an asset

DUTIES & RESPONSIBILITIES

• Provide comprehensive primary care services within the Family Care Team model
• Deliver chronic disease management, preventive care, acute care, and patient education
• Work collaboratively with nurses, nurse practitioners, and allied health professionals within the interdisciplinary team
• Participate in care coordination and transitions of care for patients requiring specialist or community services
• Contribute to quality improvement initiatives within the Family Care Team
• Support person- and family-centered care; promote patient and worker safety by integrating safety into work practices, identifying risks, and implementing improvements

REMUNERATION

Remuneration will be in accordance with the Memorandum of Agreement for salaried physicians in Newfoundland and Labrador, commensurate with level of training and experience.
