Job Summary
Works with a multi-disciplinary health team to provide primary care services to individuals and families residing in the PQCHC area using a medical care model which emphasizes a holistic approach of prevention, health maintenance, and education.
Job Specific Responsibilities
- Provide comprehensive primary medical care consistent with the standards of practice of the College of Physicians and Surgeons of Ontario (CPSO).
- Diagnose, treat, and manage acute and chronic medical conditions across the lifespan.
- Collaborate with nurse practitioners, registered nurses, allied health professionals, and social services staff to deliver team-based primary care.
- Conduct comprehensive health assessments, develop and review care plans, and provide direct clinical services.
- Order and interpret diagnostic investigations, prescribe medications, and coordinate referrals to specialists and community resources.
- Educate clients on disease prevention, health promotion, and chronic disease management.
- Maintain accurate and timely documentation in the electronic medical record in accordance with professional and organizational standards.
- Participate in team meetings, case conferences, quality improvement, and continuing professional development activities.
- Support clients experiencing health inequities linked to the social determinants of health, such as income, housing, and employment.
Qualifications
- Doctor of Medicine (MD) degree and current licensure in good standing with the College of Physicians and Surgeons of Ontario (CPSO).
- CCFP certification or eligibility with the College of Family Physicians of Canada.
- Current CMPA coverage.
- Minimum of two (2) years of experience practicing family medicine, ideally in a primary care or community health setting.
- Demonstrated experience or strong interest in working with one or more of PQCHC's priority populations: newcomers and refugees, older adults, 2SLGBTQ+ and gender-diverse clients, and people experiencing health inequities linked to the social determinants of health, such as income, housing, and employment.
- Experience in a community setting desirable.
- Demonstrated commitment to and knowledge of community-based health care.
- Technology proficiency an asset – PS Suite EMR, Microsoft Office Suite.
- Flexible hours, including some evenings.
- Vehicle and license requirements – Class G.
- Excellent communication and interpersonal skills.
- Linguistic profile: Bilingual English/French is an asset.
- Other languages an asset.
