The Program:There is an exciting opportunity to become involved in the provision of outpatient care to older adults in an interdisciplinary team environment. The Geriatric Evaluation and Management Program assists older adults to optimize independence, functioning and quality of life by assisting them to achieve the higher degree of health that is required to live within the community. The program provides a patient and family centered approach specializing in comprehensive geriatric and interdisciplinary assessment, treatment, care planning, health promotion and rehabilitative therapies to older adults living in the community. Qualifications:
Successful applicants will hold certification or eligibility for certification in Family Medicine with the College of Family Physicians of Canada and be eligible for licensure with the
College of Physicians and Surgeons of Saskatchewan (CPSS). Candidates must obtain privileges with the Saskatchewan Health Authority (SHA) to practice family medicine in Saskatoon.
