Looking for Family Physicians who seek to

• work in a highly supported environment with a team based approach to care
• provide comprehensive primary care with a focus on prevention, early detection, chronic disease management, and lifestyle oriented, evidence based and personalized medicine 
• leverage AI and othertechnologies to enhance patient care 
• seek a small panel size of 400-600 patients

Applicants should have 

• At least 3 years of clinical, primary care experience 
• CPSO licensure and CCFP certification 
• Experience in a collaborative, team-based primary care setting and in chronic disease management
