Job Description
Locum opportunity for family physicians interested in caring for older adults (no COE designation required) in Markham/Stouffville, for 12 months starting April 2027
Team: 4 collegial and supportive family physicians, team coordinator, nurse navigator x 2 (triages urgent call/visit requests), advanced practice nurse (palliative care training, home visits and intakes), community paramedics (conducts routine and urgent home visits and diagnostics for patients), home care coordinator, pharmacist, chiropodist)
Practice: Mix of virtual and in-person home visits, providing primary care to patient’s aged 65+ who cannot access office-based services in Markham (specific catchment area is north Markham and Stouffville: 404 to Markham Rd, north of 16th Avenue), twice monthly virtual interdisciplinary rounds with allied health
Roster Size: 96 patients
EMR: Accuro
Coverage: 4-4.5 days a week of virtual/in-person visits with flexibility in scheduling. 0.5-1 day a week of admin time.
—> Program hours M-F 8:30 am to 4:30 PM, once a week after hours virtual call (5-8 PM)
Compensation: Paid through alternative funding plan (AFP) salary based on roster size (Gross ~$360K+/year, 20% shadow billing and 100% after hours, minimal overhead (<5%), 4 weeks of paid vacation/year. Fee for service cap applies.
