Highlights of the Family Physician Position

(Multiple Positions Available - Up to 1.5 FTE - St.Catharines and/or Fort Erie) :

• De dwa da dehs nye>s Aboriginal Health Centre (DAHC) is recruiting for multiple Family Physician positions, with priority consideration given to full-time applicants, followed by part-time applicants.  
• Our St. Catharines Clinic, located at 145 Queenston Street, is DAHC’s newest primary care site and is actively accepting new patients. 
• We are also engaged in a new partnership to support a satellite clinic in Fort Erie, and are seeking a Family Physician interested in supporting care in Fort Erie starting 1 day per week, with the remainder of their practice based in St. Catharines.  
• Within the Primary Care Team, the Family Physician will work as part of an interdisciplinary care team that includes two Nurse Practitioners, Registered Nurse, Registered Practical Nurse, Social Worker, and Medical Administration Assistants 
• Roster sizes are adjusted to support relationship-based, comprehensive care that reflects complexity and needs of the patients we serve.
• Additional integrated services include Patient Navigation, Mental Health & Addictions, Palliative and End of Life care Programming, Traditional Programming, FASD Assessment & Diagnostics Program 
• Our clinic prioritizes cultural safety and team-based care to support patients with complex health and social needs
• Patient visits at DAHC are appointment-based and the Family Physician will provide care to DAHC’s rostered patients.

• DAHC reimburses annual professional College registration fees for all regulated health professionals.
• Clinic Hours are:
  • Mon 8:30 – 4:30
  • Tues 8:30 – 6:00
  • Wed 8:30 – 4:30
  • Thurs 8:30 – 4:30
  • Fri 8:30 – 3:00
