A well-established Family Health Organization (FHO) physician is seeking physician to take over an existing practice within a collegial group environment and a high-functioning Family Health Team by Oct 1, 2026

Practice & Team Highlights

• FHO physician in a group of 10 physicians
• Shared after-hours/on-call coverage: approximately 3 shifts per month
• Integrated with a high-functioning Family Health Team, with strong access to allied health services and patient programs
• Approx. 1,400 rostered patients
• Mixed demographic patient population
• TELUS PS Suite EMR
• Fully staffed office (experienced administrative support in place)

Clinic Location & Set-Up (Primary Site)

1338 Fourth Avenue, Unit 221, St. Catharines, ON

• Located on the 2nd floor of the Pathstones Mental Health Building
• Close to the Marotta Family (St. Catharines) Hospital
• Fully furnished with medical and office equipment
• 5 exam rooms

• Lower rental cost: ~$400/month

• On-site: independent Nurse Practitioner working with the team

Ideal Candidate

• CPSO-licensed Family Physician eligible to work in Ontario
• Comfortable in an FHO/group model and participating in shared after-hours coverage
• Confident with TELUS EMR (or willing to learn quickly)
