Reporting to the Operations Manager, the role of the Physician is to provide diagnosis and treatment, health protection and promotion, coordination of care and advocacy on behalf of clients, carrying all of these roles out in the office as well as in clients’ homes when necessary. The Physician will provide first-line medical services and refer to secondary and tertiary care within the CHC catchment area.
MAIN ACTIVITIES
Duties and Responsibilities
- Provide primary care to individuals and their families by taking into account risk factors (environmental, social, psychological and medical) in a manner which can be understood by the client, and when applicable their families.
- Work closely with the health care team to establish, implement and evaluate programs to target health needs of clients in the catchment area.
- Ensure appropriate, comprehensive treatment is delivered by maintaining complete and accurate medical records.
- Consult on the administration of the health centre by identifying areas where the development of protocols and procedures is needed to improve client services.
- Promote effective staff functioning by participating in committee work and staff meetings.
- Act as a medical resource for the CHC community.
- Mentoring and training medical and nursing students as needed.
- Provide client education on preventative health care and other health issues.
- Act as a resource and advocate for clients and families with regards to interpreting information, supporting decisions and client safety and care
- Manages service delivery in conjunction with clients, their family members/caregivers/SDMs, and other service providers/agencies, via telephone, home visiting, office visiting and written correspondence.
- Maintains professional competence and knowledge of current practices by participating in professional development and on-going research.
- Works as a member of a multi-disciplinary team and in collaboration with services in the community, and family members, wherever appropriate, for the purpose of consolidating and coordinating client treatment.
- Establishes therapeutic relationships with clients, their family members/caregivers/SDMs in order to build trust and effectively deliver services.
- Protects client privacy and confidentiality in accordance with legislation, CHC policies and standards established by the CPSO
- Develop and implement charting tools, keeping record of appropriate statistics.
- Participate in chart reviews and discussion of problem cases.
- Maintain clinical records and document client assessment findings, goals, treatment plans and intervention in accordance with CHC policies and procedures and as per guidelines established by the CPSO
- Uses sound professional judgment and acts in accordance with all CHC policies and procedures and standards of practice set forth by CPSO and other respective colleges
- Support colleague vacation by covering patient care and reviewing lab results / inbox as needed
JOB REQUIREMENTS
Education:
- Medical degree and family medicine residency from a recognized university.
- Registration in good standing with the Ontario College of Physicians and Surgeons.
Knowledge/Experience:
- Knowledge and experience working with populations with mental illness and working with diverse and marginalized populations an asset.
- Broad knowledge, experience and interest in the practice of family medicine.
- Experience in primary care setting (community-based preferred).
- Knowledge of the South Georgian Bay Community is considered an asset.
Skills:
- Demonstrated strong communication skills in English, both written and verbal.
- Demonstrated computer literacy.
- Demonstrated ability to organize, prioritize and multitask efficiently.
- Strong interpersonal skills to work with diverse client groups with varying levels of comprehension and language capability demonstrating empathy, compassion and cultural sensitivity.
- Effective listening, observation and facilitation skills to effectively assess the client and provide appropriate treatment and consultation in an inclusive manner.
- Ability to deal with demanding interpersonal situations and respond with good judgment and understanding.
- Ability to analyze and independently problem solve using a health equity lens.
- Demonstrated ability to work independently and as a member of a multi-disciplinary health services team.
- Ability to think critically and be innovative to create/modify strategy or intervention techniques to suit the individual needs of clients in an inclusive manner.
- Proficiency in French is an asset.
- Excellent organizational, time management skills and demonstrated ability to be flexible with the ability to adapt quickly to the changing needs of the centre’s programs and operation.
- Demonstrated advanced knowledge in consultation and ethical decision-making.
- Demonstrated use of theory and evidence to advance clinical practice and outcomes.
- Strong mediation, negotiation and conflict resolution skills to respond or assist in emergency situations
- Ability to engage with clients, staff and community partners in a virtual environment using appropriate technology supported by SGBCHC.
- Ability to adapt quickly to a changing health care environment.
- Valid Ontario driver’s license and use of own vehicle for CHC business
