Community Health Physician – Jane Trethewey & Lawrence Heights Sites
Part-time, Locum Position (October 2025 to October 2026)
Salaried - $181.57 per hour for 26 hours per week

Unison Health and Community Services is a non-profit, community-based organization that serves neighbourhoods in Northwest Toronto. Operating out of six locations, Unison offers core services that include primary health care, counselling, health promotion, Early Years programs, legal services, harm reduction programs, housing assistance, adult protective services, Pathways to Education™ and a Diabetes Education Program. Unison’s mission is working together to deliver accessible and high-quality health and community services that are integrated, respond to needs, build on strengths and inspire change.

Reporting to the Clinical and Allied Health Site Manager(s) for the Jane-Trethewey and Lawrence Heights sites, the Community Health Physician will be responsible for providing primary care to clients and their families within a multi-disciplinary, collaborative care model that emphasizes health promotion and client education. The Community Health Physician will have strong clinical and interpersonal skills to complement our team.
Qualifications:
· License to practice medicine and membership in good standing with the College of Physicians and Surgeons of Ontario
· Demonstrated experience in community-based primary care settings
· Demonstrated knowledge of and sensitivity to the impact of social, economic, environmental and cultural issues on health
· Ability to work as part of an interdisciplinary team of clinical and program staff
· Ability to do venipuncture or willingness to learn
· Demonstrated experience in community-based primary care settings
· Ability to manage all aspects of reproductive health including insertion and removal of IUDs
· Prescribe relevant medications and provide follow up care to clients requesting medical abortions
· Ability to initiate and provide hormone therapy to trans clients
· Experience serving Unison’s priority populations (seniors with complex needs, people with mental health and addictions, LGBTQ2IA+, and youth facing barriers)
· Commitment to working from a harm reduction, community development perspective
· Ability to speak a second language an asset
· Availability to work Mondays, Wednesday evenings, Thursday mornings, and Fridays.
