DIVISION HEAD, Community, Primary Care and Long Term Care 

Full-Time / Part-Time / Associate Appointment Opportunities 

Lakeridge Health | Durham Region, Ontario

Reporting to the Department Chief and Medical Director, Family and Community Medicine, the Division Head will represent the Community, Primary Care & Long-Term Care Division across all Lakeridge Health sites. This leader will work collaboratively with the Department Chief and Medical Director, as well as with Program and operational leadership teams, to oversee clinical operations, medical quality of care, recruitment, human resource planning, physician engagement, and academic activities across the Division. 

This role offers the opportunity to strengthen integration across hospital, primary care, community, and long-term care sectors while helping shape innovative, equitable, and patient-centered models of care across Durham Region. 

Specific responsibilities include: 

Clinical Operations 

  • Keep abreast of provincial context and regional expectations for community, primary care, and long-term care services 
  • Act as a two-way conduit with Program leadership and the Division/Department 
  • Provide input into the development of a strategic plan for the Division along with its implementation 
  • Support effective integration, operational efficiency, best practice management, and patient-centred care 
  • Support collaboration and relationship-building across hospital, community, primary care, and long-term care partners 

Quality of Care 

  • Lead development of the Quality Agenda for the Division, including annual goals and objectives 
  • Support the identification, implementation, and measurement of evidence-based practices 
  • Establish and communicate clear performance expectations 
  • Lead and ensure Division Patient Safety and Quality Rounds occur 
  • Participate in Quality of Care Reviews when requested by the Department Chief and Medical Director 
  • Help ensure a collegial, safe, respectful, and diverse culture 

Professional Practice & Performance Management 

  • Support recruitment, credentialing, and performance reviews in collaboration with the Department Chief and Medical Director 
  • Identify, collaborate, and participate in complaints and incident investigations, management, and responses, as well as recognition and acknowledgement of credentialed staff 
  • Support the Department Chief in evaluating and managing professional practice issues 
  • Support, coach, and mentor others to achieve their best 

Administrative Responsibilities 

  • Support the Mission, Vision, and Values of Lakeridge Health 
  • Ensure regular two-way communication with Credentialed Staff 
  • Facilitate the advancement and integration of Inclusion, Diversity, Equity, Accessibility, and Anti-Racism (IDEAA) 
  • Support the completion of human resource scheduling, workload, and wellness initiatives within the Division while working collaboratively with Department and operational leaders 
  • Participate in additional committees as appropriate and defined by the Department Chief and Medical Director

Qualifications 

Required 

Applicants must: 

  • Hold, or be eligible for licensure, with the College of Physicians and Surgeons of Ontario (CPSO), including eligibility for an Independent Practice Certificate or a Restricted/Provisional certificate as applicable.  
  • Be certified, or eligible for certification, in CFPC / other relevant body  
  • Be eligible for hospital privileges at Lakeridge Health  
  • Minimum two years of clinical experience 
  • Work effectively in a collaborative, team-based environment  
  • Intention to complete IDEAA modules 
  • Ability to manage competing demands and engage constructively with people 
  • Strong communication, collaboration, and relationship-building skills 

Preferred / Assets 

Preference may be given to candidates who demonstrate: 

  • Interest or experience in medical education, teaching, or preceptorship  
  • Interest or experience in quality improvement or program development  
  • Interest or experience in primary care, community medicine, long-term care, or system integration 
  • Interest in integrated care, community partnerships, and physician leadership
