Job Description

POSITION TITLE: Family Physician (Full-Time) LOCATION: Waterloo, Ontario, Fee-for-Service

JOB SUMMARY The Family Physician provides comprehensive, continuous, and patient-centered primary healthcare services to individuals and families in the community of Waterloo, Ontario. Operating in accordance with standards established by the College of Physicians and Surgeons of Ontario (CPSO), the Ontario Ministry of Health (MOH), and the College of Family Physicians of Canada (CFPC), the physician is responsible for acute medical care, chronic disease management, health promotion, and disease prevention.

CORE RESPONSIBILITIES AND DUTIES

  1. Clinical Practice & Patient Care
  • Provide continuous and comprehensive primary medical care to a assigned panel of patients across all life stages.
  • Diagnose and manage acute illnesses, complex chronic conditions (e.g., diabetes, hypertension, asthma, mental health conditions), and deliver routine health maintenance assessments.
  • Formulate evidence-based care plans, prescribe medications in alignment with Ministry of Health policies and the Ontario Drug Benefit (ODB) formulary.
  • Provide virtual care assessments when clinically appropriate, adhering strictly to the CPSO Virtual Care Policy.
  1. Diagnostic & Continuity Management (CPSO Compliance)
  • Review, track, and act upon diagnostic test results (laboratory, diagnostic imaging, pathology) in a timely manner as required by the CPSO Managing Tests Policy.
  • Manage referrals to specialists and secondary/tertiary facilities, ensuring complete and timely care transitions as per the CPSO Transitions in Care Policy.
  • Participate in the clinic’s shared after-hours coverage roster and call schedule to ensure continuous patient care access, adhering to the CPSO Availability and Coverage Policy.
  1. Medical Records & Administration
  • Maintain complete, accurate, and timely Electronic Medical Records (EMR) for all clinical encounters in compliance with the Personal Health Information Protection Act (PHIPA) and the CPSO Medical Records Documentation Standard.
  • Complete required administrative and medical-legal documentation (e.g., MOH forms, disability reports, insurance documents, driver's assessments).
  1. Interprofessional Collaboration & Quality Improvement
  • Work collaboratively within a multidisciplinary team environment, including registered nurses, nurse practitioners, allied health professionals, and administrative staff.
  • Coordinate with local healthcare partners, including the region's Ontario Health Team (OHT) and area hospitals (e.g., Grand River Hospital, St. Mary's General Hospital).
  • Engage in Infection Prevention and Control (IPAC) practices as outlined by CPSO/Public Health guidelines.
  • Participate in clinic-level Quality Improvement (QI) activities and satisfy mandatory CPSO Continuing Professional Development (CPD) requirements.

QUALIFICATIONS AND REQUIREMENTS

  • Medical Degree (MD or DO) from a recognized medical school.
  • Completion of an accredited Family Medicine Residency Program.
  • Certification or eligibility for certification by the College of Family Physicians of Canada (CCFP).
  • Active Independent Practice License (or eligibility for registration) with the College of Physicians and Surgeons of Ontario (CPSO).
  • Valid membership and medical liability coverage with the Canadian Medical Protective Association (CMPA).
  • Authorized to work in Canada (Canadian Citizen, Permanent Resident, or valid Work Permit).

WORKING CONDITIONS

  • Outpatient medical clinic setting equipped with Electronic Medical Records (EMR).
  • Regular business hours with periodic participation in after-hours and weekend call coverage as dictated by the clinic model.
