Family Physician

Multiple opportunities: Full-Time, Part-Time, Locum, Temporary, or Permanent

Rexdale Community Health Centre (RCHC) is a multi-service, community-based organization providing primary health care, health promotion, and community support services in North-West Etobicoke. We serve a diverse community of families, children, youth, adults, and seniors, and we’re committed to equity-driven, culturally safe care.

What your first week looks like:

  • Shadowing and orientation with the team and clinic workflows
  • Policies and practice expectations (CHC model, culturally safe care, equity lens)
  • EMR training and support (including results/messages workflows)
  • Clinic processes + introductions to key inter-professional supports and referral pathways


Responsibilities and Tasks

  • Provide primary care to individuals and families, taking into account risk factors (environmental, socio-economic, psychological, and medical) in a manner which can be understood by the client and, when applicable, their families.
  • Perform clinical duties including routine health and illness care, assessment and treatment planning, prescribing, and appropriate follow-up, while maintaining complete and accurate medical records.
  • Provide on-call service when scheduled, make home visits when necessary, and keep other primary care providers informed regarding client issues arising when on-call.
  • Work with the health care team to plan and evaluate health education, prevention, and screening programs based on identified health needs of clients and communities.
  • Consult other primary health care providers in relation to medical opinions and information.
  • Participate in chart reviews, case conferences, and continuous quality improvement activities.
  • Provide leadership in overseeing patient cancer screening and appropriate MSSA indicators.
  • Assist in planning and programming of centre health services, participate in committees, and share information about perceived health needs and issues in the community through client encounters and other community contacts.
  • Facilitate the administration of the health centre by identifying areas where protocols/procedures are needed to improve client services or staff functioning, and participating in committee work and staff meetings.
  • Maintain and develop professional competence through continuing education (e.g., peer interaction, literature review, conferences, courses).
  • Undertake teaching and training of medical trainees within the health centre (optional/as applicable).
  • Other duties as provided by the Director of Primary Health Care Services.


What We’re Looking For

  • MD and Family Medicine residency from a recognized university
  • CPSO license + CCFP (or eligible)
  • CMPA (or eligible)
  • Commitment to equity-driven, community-based, culturally safe care
  • Strong communication and interpersonal skills; collaborative and adaptable
  • Comfortable working in diverse settings with an interdisciplinary team
  • Knowledge of the Rexdale community is an asset
  • Additional language skills are an asset (e.g., French, Spanish, Arabic)


Location

RCHC operates across North Etobicoke:

  • 8 Taber Road
  • 222 Dixon Road
  • 21 Panorama Court
  • Plus occasional local travel for outreach/mobile services (Wheels to Wellness).


Compensation & Scheduling

  • $160.64/hour (as per OMA) plus
  • HOOPP and a generous benefit and vacation package for permanent roles
  • Locum and temporary roles are paid 10% in lieu of benefits
  • Standard hours are Monday – Friday 9am- 5pm
  • Scheduling options include daytime and evening shifts (we’re open 8:00am - 8:00pm for compressed workweek scheduling)
  • Weekend shifts available
  • Full-time: 35 hours/week
  • Part-time/locum/temporary: flexible and negotiable
