Job Description
The Carefirst Family Health Team (FHT) is seeking full-time and part-time permanent and locum family physicians to join our innovative, interdisciplinary team dedicated to delivering high-quality, community-based care.
About Us
Carefirst FHT is committed to delivering exceptional, comprehensive primary care through a collaborative, team-based model that includes family physicians, nurse practitioners, nurses, social workers, dieticians, physiotherapists, and medical receptionists. We have a robust Chronic Disease Management and Prevention program, including a Diabetes Education Program, and are actively involved in many of the Ontario Health Team’s primary care initiatives.
Our "Hub & Spoke Model" and strong community partnerships ensure coordinated, high-quality, patient‑centred care across the continuum. We work closely with the Carefirst Seniors & Community Services Association to provide truly integrated services.
On‑site & Integrated Programs/Services include:
· Ontario Health atHome (formerly Home and Community Care Support Services)
· MINT Memory Clinic
· Ontario Structured Psychotherapy (OSP) Program
· Specialist clinics: psychiatry, cardiology, endocrinology, nephrology, optometry, geriatric medicine, gynecology, chiropody, and audiology
Clinic Locations
We currently operate four well-established clinics across Scarborough, Markham, and Richmond Hill, and continue to expand in Markham to support continued growth and community needs.
Current clinic locations:
· 300 Silver Star Blvd, Scarborough (Midland / McNicoll Ave)
· 420 Hwy 7 East, Richmond Hill (West Beaver Creek / Hwy 7)
· 360 Hwy 7 East, Richmond Hill (Chalmers Rd / Hwy 7)
· 105 Gibson Centre, Markham (Steeles Ave East / Warden Ave)
Upcoming new clinic location:
· Unionville Commons Seniors Hub, Markham (Unionville Main Street/Highway 7 East)
Physicians will be assigned to a location that aligns with their preferences and residence, with occasional rotation to support clinic operations.
The Position
With this rapid expansion, we are looking for compassionate, team-oriented family physicians who are committed to delivering high-quality care and value interdisciplinary collaboration.
An existing patient waitlist offers an excellent opportunity to establish a thriving practice quickly. We welcome new graduates and international medical graduates fulfilling return-of-service requirements!

Qualifications
§ Licensed to practise as a family physician in Ontario and in good standing
§ Excellent communication and interpersonal skills, time management and work ethic
§ Team-oriented, compassionate and approachable
§ Proficiency in a second language relevant to the local population is an asset
Compensation
§ Income stabilization program option, fully funded by MOHLTC
§ Blended Salary Model position with benefits
§ Vacation leave, Sick Leave, Flex Day and Continuing Medical Educational Leave for FTP/PTP position
§ $170 per four for Locum position
