Family Health Organization (FHO+) is adding an additional Family Physician in Guelph, Ontario

A rare and exciting opportunity is available for a Family Physician to join a thriving, established Family Health Organization Plus (FHO+) in Guelph, Ontario. With a flexible start date, this well-managed and collegial group practice offers physicians the opportunity to build a new patient roster or transition an existing practice while benefiting from a mature clinical infrastructure, interdisciplinary support, and significant growth potential in one of Ontario's fastest-growing communities.

Practice Opportunity

The successful candidate will join a team of six family physicians within an established and highly regarded clinic located in central Guelph. Physicians enjoy flexibility in scheduling, with the capacity to provide up to seven half-days of clinic per week and can comfortably support a roster of up to  1,500 to 1,750 patients. This opportunity is ideal for candidates looking to establish a new practice, grow an existing patient roster, or transition patients into a supportive Family Health Organization Plus (FHO+) environment.

Practice Highlights

·        Opportunity to join a mature, well-organized family practice

·        Establish and/or bring over your patient roster, with the ability to handle up to 1750 patients

·        FHO on-call rotation requires 1-2 evenings per month and 1-2 weekends per year

·        11 patient rooms (1 dedicated to FHT Nurse Practitioner and 1 additional for Primary Care Nurse Clinician)

·        Affiliated with a Family Medicine Residency Program

·        Established physician group with more than 25 years of service in the community

·        Clinic hours: Monday thru Friday - 9:00am - 5:00pm

·        Additional Hospitalist opportunities available at Guelph General Hospital

As part of a Family Health Team, physicians have access to a comprehensive network of allied health professionals and administrative staff, including:

This collaborative model allows physicians to provide comprehensive, patient-centered care while benefiting from strong clinical and administrative support.

Established and supporting Guelph and surrounding communities since 2006. Guelph FHT offers team-based care with nurse practitioners, primary care nurse clinicians, registered dietitians, mental health counsellors, and clinical pharmacists. They have access to Diabetes Care Guelph, INR, Psychiatry and Wellness Programs. They support over 95 family doctors, 120,000 patients across 25 offices.
