The Huron community Family Health Team (HcFHT) located in Seaforth, ON. is currently recruiting a Full or Part Time Family Physician to join the team and assume the practice of a retiring physician.

The practice includes 1,200 rostered patients and is available as early as Fall/Winter 2025.  There is opportunity to locum in the practice and/or job share before assuming the roster, to confirm the overall fit with the practice and group.

There are 6 family physicians that make up the HcFHT, in addition to FHT providers and they are all co-located in a newer turn key clinic, conveniently located beside Seaforth Community Hospital.  The FHT staff include NPs, a Physician Assistant, RPNs, a dietician, social worker/mental health workers, and administrative staff.  

The HcFHT is a regional patient-centred, primary health care team set up to provide collaborative health care to improve the overall health of the Huron East population.

Family physicians within the HcFHT do participate in hospitalist and/or emergency work at Seaforth Community Hospital (SCH).  SCH is a 20 bed hospital, with a mix of medicine, complex continuing care and rehabilitation beds and is located in close proximity to secondary and tertiary referral centres - Stratford General Hospital and London Hospitals.

PACS, X-Ray, Lab & U/S are available on site.

Hospitalist physicians are scheduled for one week durations (Fri-Thurs).  Physicians receive a daily stipend for their hospitalist work.

Physicians at the HcFHT are eligible for funded locum support through the Rural Family Medicine Locum Program.  This program provides 21 funded locum days per year, per physician.

The community of Seaforth is located less than one hour from London and Kitchener and 30 minutes from Stratford.  The beaches of Lake Huron are located only a short drive away.  This opportunity qualifies for the MoHLTC's Northern Rural Recruitment and Retention initiative in the amount of $86,335 for full time candidates.  Additional incentives are available through the Seaforth Community Hospital Foundation and support physicians providing comprehensive patient care.
