INTRODUCTION:
De dwa da dehs nye>s Aboriginal Health Centre’s mission is to improve the health and well-being of Indigenous individuals, families and communities through wholistic Indigenous, Traditional and Western health care. As an Aboriginal Health Access
Centre, we provide a wide range of health and social services in the Hamilton, Brantford and Niagara region including Primary Care, Mental Health & Addictions Services, Health Promotions, Traditional Healing and Programs, Advocacy and Outreach, Palliative & End of Life Supports and much more in partnership with local community organizations.
De dwa da dehs nye>s Aboriginal Health Centre (DAHC) is recruiting for a Family Physician position, with priority consideration given to full-time applicants, followed by part-time applicants. Candidates must be willing to provide care at a satellite clinic in
the surrounding Brantford area starting 1 day a week, with remainder of practice based in Brantford.
Within the Primary Care Team, the Family Physician will work as part of an interdisciplinary care team that includes Nurse Practitioners, Registered Nurse, Registered Practical Nurse, Social Worker, and Medical Administration Assistants.
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:
- Provide direct patient care to a roster of patients across the lifespan, delivering accessible, culturally safe, comprehensive, and high-quality primary care services.
- Work within an integrated model of care that includes Patient Navigation, Mental Health & Addictions, Palliative and End of Life Care, Traditional Healing and Wellness Programming, and the FASD Assessment & Diagnostics Program.
- Support a clinic environment that prioritizes cultural safety, relationship-based care, and interdisciplinary collaboration for patients with complex health and social needs.
- Deliver comprehensive primary care with a focus on health promotion, disease prevention, chronic disease management, and acute/episodic care.
- Demonstrate willingness and ability to provide occasional home visits for patients who experience barriers accessing clinic-based care.
- Apply an understanding of wholistic approaches to health and wellness, with openness and respect for the unique experiences, beliefs, and needs of each individual.
- Participate in cross-coverage for other clinicians as needed to support continuity of care.
- Collaborate effectively as part of an interdisciplinary care team.
STATEMENT OF QUALIFICATIONS:
- Maintains competence and knowledge relevant to current clinical practices and guidelines
- Maintains professional development requirements and license with CPSO in good standing
- 3-5 years’ work experience in a health care setting in a direct care provision role. Experience working in a primary care setting an asset.
- Experience working with Indigenous community members an asset.
- Proficient in navigating electronic medical records and maintaining records in a timely, efficient and effective manner
- Knowledge of First Nations, Metis and Inuit culture and communities, as well as the social determinants of health that have the greatest impacts
- Knowledge of and practical application of relevant privacy and confidentiality legislation
- Ability to communicate effectively and professionally, both written and verbal
- Strong interpersonal skills and ability to work both as part of a team and independently
- Clear vulnerable sectors check is required
- Valid driver’s license and reliable vehicle
OTHER INFORMATION:
Hourly Salary: $182.40
Office hours: 8:30-4:30PM on Monday, Tuesday and Thursday, 8:30-6:00PM on Wednesday and 8:30-3:00PM on Friday
