Internal referrals amongst our physician group for IUD insertion/removal, OB delivery/prenatal care, toe nail removals, HRT, nurse who does derm procedures. etc and much more

FHO roster is available for a fee (please contact directly for inquiries). 1200 patients. Roster is currently generating revenue of $37,000-$40,000 per month with the new FHO+ model.

Very heavy diabetic practice (out of the basket), currently all synchronized into one day per week (weds) every 3 months.

OVERHEAD COSTS

Overhead costs are minimal due to costs being capped at max $7500 per month which works out to be only approx 20% OFFICE OVERHEAD! (30% of current monthly payment capped at $7500 works out to be 20%)

3 Nursing staff employed to help complete forms, do pap smears, vaccines, vitals, outbound calls to patients, INR tracking, cognitive testing and much more.

Office manager/biller takes care of all of your billing needs including billing the FHO+ hours, just write your diagnosis and code and leave your daysheet with her! Also does your tracking for preventative screening (pap smear, vaccines, colorectal screening etc)

5 admin staff responsible for checking in patients, scheduling follow ups and more

Patient reminders in place through Ocean, appointment notifications go out 1 week prior to their appointment date.

Just built in 2019 - it contains a Lifelabs, Georgian Radiology for Xray/Ultrasounds, Myodynamic Physio, Pharmacy, RVH satellite clinic for ECG/PFTs, General Surgeons and OBGYN Specialists etc.

REASON WHY I AM REQUESTING A TAKE-OVER?

I have moved up north and joining the Orillia FHO and starting a new practice to be closer to home.

I will help you join the Barrie FHO/FHT and ensure the transition is smooth. I will take care of the letters to the patients notifying them of the transition. I can also roster new patients at the request of the physician during the transition period.
