ROLE SUMMARY
━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━
As a Family Physician at St. George Medical Center, you will provide comprehensive, continuous primary care to a rostered panel of patients across all ages and life stages — from pediatric to geriatric. You will work within the Holly-Thompson FHO framework, supported by our clinical and administrative team, to deliver evidence-based, patient-centred care.

KEY DUTIES & RESPONSIBILITIES
━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━
Clinical Care
• Provide comprehensive family medicine care to rostered patients of all ages including infants, children, adults, and seniors
• Conduct scheduled appointments including annual physicals, well-baby visits, prenatal care, and follow-up consultations
• Diagnose and treat acute illnesses, injuries, and urgent medical conditions in both scheduled and walk-in settings
• Develop individualized care plans based on patient history, clinical findings, and best practices
• Order and interpret diagnostic investigations including laboratory tests, imaging, and ECGs
• Prescribe medications in accordance with current clinical guidelines and Ontario drug benefit programs
Preventive Care & Health Promotion
• Perform health screenings, immunizations, and preventive care procedures in line with Ontario public health guidelines
• Administer and oversee the clinic's vaccination program including influenza, COVID-19, Tdap, MMR, Hep B, HPV, and Shingrix
• Counsel patients on disease prevention, healthy lifestyle choices, nutrition, mental health, and smoking cessation
• Conduct cancer screening (colorectal, cervical, breast) and follow up on abnormal results
Chronic Disease Management
• Manage patients with chronic conditions including diabetes, hypertension, asthma/COPD, heart disease, thyroid disorders, and obesity
• Develop and monitor individualized chronic disease management plans using evidence-based protocols
• Coordinate care with allied health professionals, specialists, and community resources
• Monitor and review lab results, adjust treatment plans, and ensure timely follow-up
Mental Health
• Screen, assess, and provide initial management for common mental health conditions including depression, anxiety, and ADHD
• Provide brief counseling and supportive care; refer to mental health specialists as clinically appropriate
• Complete mental health forms and documentation required for disability, WSIB, and insurance purposes
Referrals & Specialist Coordination
• Generate and manage referrals to specialists, ensuring timely follow-through and communication
• Review specialist consultation notes and integrate recommendations into patient care plans
• Advocate for patients to ensure access to appropriate specialist and hospital care
Documentation & Compliance
• Complete accurate, timely EMR documentation for all patient encounters within 24 hours of visit
• Ensure proper OHIP billing and shadow billing in compliance with FHO model requirements
• Maintain patient records in compliance with PHIPA (Personal Health Information Protection Act)
• Complete required medical forms, letters, sick notes, WSIB forms, disability documentation, and insurance reports
• Participate in quality improvement activities and comply with CPSO standards of practice
Team Collaboration
• Work collaboratively with clinic physicians, nursing staff, and administrative team
• Participate in regular team meetings and clinical case discussions
• Mentor and support clinical staff in patient care procedures as needed
• Contribute to clinic policies, protocols, and quality improvement initiatives
• Comply with all infection prevention and control (IPAC) standards and Ontario public health regulations
After-Hours Responsibilities (FHO Requirement)
• Participate in the FHO after-hours call rotation to ensure rostered patients have access to care outside regular clinic hours
• Provide telephone advice and triage to patients after hours as required under the FHO model
━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━
REQUIRED QUALIFICATIONS
━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━
• Medical degree (MD or equivalent) from a recognized institution
• Completion of a recognized Family Medicine Residency Program
• CCFP designation (or eligibility — new graduates welcome to apply)
• Current, independent license with the College of Physicians and Surgeons of Ontario (CPSO)
• Active membership with the Canadian Medical Protective Association (CMPA)
• Eligible to bill OHIP (Ontario Health Insurance Plan)
• Strong clinical knowledge across the full scope of family medicine
• Proficiency with or willingness to learn EMR systems (experience with OCEAN / Cognisant MD an asset)
• Excellent communication, interpersonal, and organizational skills
• Commitment to patient-centred, ethical, and culturally sensitive care

PREFERRED QUALIFICATIONS
━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━
• Experience in a Family Health Organization (FHO) or similar team-based care model
• Interest or experience in women's health, pediatrics, geriatrics, or chronic disease management
• Bilingual (English + any additional language reflecting our diverse Milton community is an asset)
• Experience with walk-in clinic practice
• Interest in teaching or supervising medical students or residents

COMPENSATION
━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━━
• Competitive compensation under the FHO model (base capitation + fee-for-service + bonus incentives)
• Overhead arrangement to be discussed — competitive with Milton and Greater Toronto Area market rates
• Overhead includes: fully staffed clinic, EMR, billing support, reception, exam rooms, and all clinical supplies
• Additional income through walk-in shifts available

WORK ENVIRONMENT

• After-hours call: Shared rotation among FHO physicians
