Family Physician
We are currently seeking dedicated and patient focused family physicians to join our established primary care clinic. This is an excellent opportunity to join a collaborative and supportive clinical environment with strong nursing, administrative and management support with an existing patient demand. The Physician will provide comprehensive, continuous, and patient centred primary care to individuals and families within a team-based practice.
Key Responsibilities
· Provide comprehensive primary care to patients across the lifespan, including preventive, acute, chronic, and palliative care.
· Assess, diagnose, and treat a broad range of medical conditions using evidence-informed clinical judgment.
· Order, interpret, and follow up on appropriate investigations, diagnostic imaging, and specialist recommendations.
· Develop and monitor treatment plans, prescribe medications, and support chronic disease management.
· Promote continuity of care through follow-up, referrals, care coordination, and smooth transitions between community and hospital settings.
· Maintain accurate and timely clinical documentation and contribute to quality improvement and patient safety initiatives.
· Collaborate effectively with allied health professionals and participate in interprofessional case management when appropriate.
Practice Environment
This is a collaborative practice which is part of a Family Health Organization consisting of 21 physicians. We are supported by a Family Health Team located within our building which allows our patients access to social workers, dietitians, a pharmacist, nurse practitioners, an occupational therapist, a respiratory therapist, and nurses. The clinic is located along the waterfront of Georgian Bay in Owen Sound and offers a well-equipped facility with procedural rooms, on-site diagnostic imaging for plain films and ultrasound, and access to a pharmacy, dental, physiotherapy, oxygen therapy, and lab services. We offer opportunities to teach through the two-year family medicine residency program at McMaster University and work with students from other universities.
Qualifications
· Medical degree and completion of accredited family medicine residency training.
· Licensed or eligible for licensure to practise medicine in Ontario through the College of Physicians and Surgeons of Ontario.
· Certification in Family Medicine from the College of Family Physicians of Canada, or eligibility to obtain certification.
· Commitment to team-based, patient-centred care and strong communication and collaboration skills.
· Interest in teaching, chronic disease management, or expanded scopes of practice is considered an asset.
