Seeking a permanent Family Physician to join our team.
The program provides primary care house calls to frail seniors in Vancouver. Doctors are part of an interdisciplinary team providing care to a panel of patients. There is some flexibility in terms of time commitment with the opportunity to take on a full time or part time role.
Primary responsibilities include managing patient care on a day to day basis, including chronic disease management and palliative care. This includes house calls, virtual visits, managing medications, monitoring labs, corresponding with allied health providers, and being available for more urgent issues as they arise.
The doctors are part of a one in nine weekly call group, providing after hours and weekend on-call service. The team is committed to reducing hospitalization and supporting end of life care in the home according to patient and family wishes.
Applicants should have an interest in frail elder care and enjoy working in a close knit team environment. A personal vehicle for travelling to house calls is required.
The practice is supported by VCH funded office in Vancouver with secretarial support. The physician payment will be primarily fee for service according to the current MSP fee schedule.
Key Accountabilities:
• Provide longitudinal patient and family centred care
• Develop collaborative trusting professional relationships with interdisciplinary team members, health authority staff and community physicians.
• Attend rounds and /or huddles
• Communicate clearly with patients, families and other health care providers.
• Initiate and engage with patients and family members in Goals of Care discussions
• Participate in after hours call group and be willing to provide timely house calls to appropriate clients when needed
• Partner with Home and Community Health to promote team-based care to prevent unnecessary ER visits and admissions.
