Community Health Physician – Oakwood-Vaughan Site
Part-time, Permanent Position – 14 hours per week
Tuesday evenings and Thursday days
Salaried - $132,182.71 per annum for 14 hours per week

Unison Health and Community Services is a non-profit, community-based organization that serves neighbourhoods in Northwest Toronto. Operating out of six locations, Unison offers core services that include primary health care, counselling, health promotion, Early Years programs, legal services, harm reduction programs, housing assistance, adult protective services, Pathways to Education™ and a Diabetes Education Program. Unison’s mission is working together to deliver accessible and high-quality health and community services that are integrated, respond to needs, build on strengths and inspire change.

Oakwood-Vaughan (OV) is the newest of six Unison sites. The OV site offers youth specific mental health, addictions and sexual health care in addition to Primary Care services.
At this time, Unison is seeking a Community Health Physician for the Oakwood-Vaughan site. Reporting to the Clinical and Allied Health Site Manager for Lawrence Heights and Oakwood Vaughan, and with direction from the Clinic Coordinator, the Community Health Physician will be responsible for providing primary care to clients and their families within a multi-disciplinary, collaborative care model that emphasizes health promotion and client education. They will also provide walk-in care to youth in areas of sexual and reproductive health, mental health and addictions. The Community Health Physician will have strong clinical and interpersonal skills to complement our team.
Qualifications:
· License to practice medicine and membership in good standing with the College of Physicians and Surgeons of Ontario
· Availability to work one evening per week
· Demonstrated experience in community-based primary care settings
· Demonstrated knowledge of and sensitivity to the impact of social, economic, environmental and cultural issues on health
· Ability to work as part of an interdisciplinary team of clinical and program staff
· Ability to do venipuncture or willingness to learn
· Demonstrated experience in community-based primary care settings that focus on youth
· Ability to manage all aspects of reproductive health including insertion and removal of IUDs
· Prescribe relevant medications and provide follow up care to clients requesting medical abortions
· Interest in or ability to initiate and provide hormone therapy to trans clients
· Maintain professional development related to youth, sexuality, and related specific job responsibility issues
· Demonstrated commitment to equity goals and principals, to sexual and reproductive rights, to being youth positive, LGBTQ2IA+ positive, sex positive, and to being inclusive of diverse sexual and gender identities
· Commitment to pro-choice philosophy
· Experience serving Unison’s priority populations (seniors with complex needs, people with mental health and addictions, LGBTQ2IA+, and youth facing barriers)
· Commitment to working from a harm reduction, community development perspective
· Ability to speak a second language an asset.
