Location: East End Community Health Centre, 1619 Queen Street East, Toronto ON

Hours of work: 0.56 FTE (based on 37.5 hours/week) – three days/week, one evening shift/week, one Saturday/month, on-call rotation

Salary: $331,968 for 1.0 FTE or $170.24 hourly

POSITION SUMMARY

The Primary Care Physician provides comprehensive, client-centered, primary health care services as a member of the interdisciplinary team to individuals and families who may experience barriers to good health and wellness. The Primary Care Physician typically assists clients with long-term management of chronic health care conditions, including mental health issues.

The Primary Care Physician will function within their scope of practice in accordance to the standards of the College of Physicians and Surgeons of Ontario (CPSO), and to guidelines and policies established at the Centre.

RESPONSIBILITIES

·        Provide primary health care services by:

-       conducting client interviews, assessments, examinations, treatments, procedures, and other medical functions as required according to the College of Physicians and Surgeons of Ontario standards of practice;

-       ensuring appropriate follow-up treatment for clients by reviewing laboratory and diagnostic test results and coordinating client services;

-       prescribing medication as needed, and monitoring for effectiveness;

-       making home visits, as required;

-       providing referrals to other staff or specialists when required; and

-       promoting healthy lifestyle choices and health education to clients.

·        Provide on-call services once per week and one weekend per month

·        Actively participate within an inter-professional model of care.

·        Provide/receive consult on client care to/from other members of the interdisciplinary team

·        Maintain complete and accurate client electronic medical records according to the standard of medical records at the Centre, including assuring privacy and confidentiality.

·        Contribute to the development, implementation, monitoring, and evaluation of medical protocols and directives.

·        Participate in clinical team audits and other quality improvement, accreditation and best practice activities

·        Inform and support the development, implementation, and evaluation of health education and promotion programs and activities as needed, in response to community health team’s requests and client needs.

·        Facilitate the administration of the health centre by supporting the development of protocols, policies or procedures to improve client services or staff functioning. Participate in staff meetings as required.

·        Advocate on behalf of clients to meet basic health needs and identify advocacy issues to support and inform the board’s effort to advocate for healthy public policies

·        Mentor, teach, and evaluate students at the Centre, as appropriate

·        Maintain and develop professional competence as defined by CCFP through appropriate continuing education opportunities, including accredited activities, self-reflection, and performance review, in consultation with Director, Clinical Services and Quality Improvement, share learnings with all team members

QUALIFICATIONS

·        Medical degree and family medicine residency from a recognized university

·        Licensed to practice medicine in Ontario

·        Must be currently registered and in good standing with the College of Physicians and Surgeons of Ontario

·        Membership and Certification with the Canadian College of Family Practitioners (CCFP) is preferred

·        Current liability insurance through Canadian Medical Protective Association (CMPA)

·        Demonstrated ability, commitment to and knowledge of community health care and health promotion

·        Experience in primary care settings (community-based preferred)

·        Ability to deal effectively with people experiencing crisis and/or mental health challenges

·        Able to work independently, and to work effectively in an interdisciplinary team of clinical and program staff

·        A strong team player with excellent communication, time management, problem-solving, and conflict resolution skills

·        Excellent interpersonal skills with clients in a culturally diverse practice population

·        Demonstrated knowledge of and sensitivity to the impact of social, economic, environmental, and cultural issues on health

·        Proficiency in the use of computers and experience in using EMR (preferably PSS)

·        Second language, relevant to catchment area populations, is an asset

CONDITIONS OF EMPLOYMENT*

·        Current and ongoing registration in good standing with the applicable college or regulatory body for your practice area, if applicable

·        Proof of professional liability insurance that meets your regulatory college’s mandatory requirement, if applicable

·        A Criminal Record check, a Criminal Record and Judicial Matters Check and/or a Vulnerable Sector Check that is satisfactory to the Centre

·        Proof of tuberculosis testing and COVID-19 vaccination
