Job Type: Temporary Part-Time (0.5 FTE) (3 Month Contract)

Hiring Range: $131.42 - $149.21 per hour


Position Summary: 

Hamilton Urban Core Community Health Centre (HUCCHC) is a dynamic, community-governed primary healthcare organization dedicated to improving the health and well-being of individuals, families, and communities living within Hamilton's urban core. Through an interdisciplinary and client-centred model of care, HUCCHC provides comprehensive primary healthcare, health promotion, illness prevention, advocacy, and community development services to individuals experiencing complex health and social challenges.

Hamilton Urban Core is committed to advancing health equity and addressing the social determinants of health. Our priority populations include individuals experiencing homelessness or housing instability, newcomers, immigrants and refugees, individuals and families living with low income, people living with mental health and substance use challenges, street-involved youth, and isolated seniors. The Physician will also support Refugee Health services through the Centre's specialized Refugee Transition Clinic.

The Refugee Health Physician provides primary care functions and participates in the development, implementation, monitoring and evaluation of programs and services for individuals, families and the community with particular emphasis on care and services for clients with chronic and complex health conditions. 

Accountability: 

The Refugee Health Physician is a member of the Primary Health Care Team, is accountable to the team and reports to the Primary Health Care Manager. Overall accountability is to the Executive Director. 

Key Responsibilities: 

  • Perform assessment, diagnosis, treatment, health education/counselling and other medical functions as required according to the College of Physicians and Surgeons of Ontario standards of practice.
  •  Participate in the Centre’s on call service as directed.
  •  Contribute to the development, implementation, monitoring and evaluation of medical protocols and directives.
  • Contribute to chart documentation, participate in chart reviews and case conferences, and make referrals.
  • Maintain accurate, current client records, data and information and electronically encounter client services in accordance with the Centre’s policies, best practices and standards of good practice. Complete and submit reports as required.
  • Participate in the development, implementation, monitoring and evaluation of treatment, education counselling and health promotion programs and services for individuals, families and the community.
  •  Coordinate or be involved with clients during periods of hospitalization and with community health professionals when patients are receiving follow-up healthcare. 
  • Work with the healthcare team to plan and evaluate programs and develop or initiate health teaching, screening programs or workshops by identifying health needs of clients and various other community groups. 
  • Assist in planning the Centre’s services by participating on committees and sharing information about perceived health needs and issues, obtained through client encounters and community contacts. 
  • Actively participate in the Centre’s quality improvement and quality assurance plans. Participate in meetings of the Centre as appropriate such as Team, Case Conference and Staff meetings.
  • Engage and provide supervision to graduates, students and/or learners as appropriate and in accordance with the Centre’s standards and commitment to learning 
  • Ensure appropriate, comprehensive treatment is delivered by maintaining complete and accurate medical records, participating in chart reviews and discussion of problem cases.
  •  Maintain and develop professional competence.
  • Work within an anti-racism, anti-oppression, culturally sensitive, language appropriate framework
  • Work as part of the interprofessional team at the centre and adhere to the Centre’s core competencies and the CHC Model of Care 
  • Participate in Centre-wide activities such as the annual general meeting, client-centered activities, annual events and all other special events and activities related to Hamilton Urban Core  

Performance Targets: 

The approximate number of clients served will be 10 to 12 per day based on an average 7.0 hour work day for a 1.0 full-time Physician position. The average appointment length will be 30 minutes with the exception of instances where a cultural interpreter is engaged or when home visits, examinations and/or procedures require more time.

Qualifications:

  • A Doctor of Medicine (M.D.) degree from an accredited institution
  • Successful completion of a recognized Family Medicine residency program.
  • Current CCFP designation in good standing
  • Are you currently registered with, the College of Physicians and Surgeons of Ontario.
  • Demonstrated competence in comprehensive primary care, including assessment, diagnosis, treatment, chronic disease management, health promotion, preventive care, and coordination of care for individuals and families across the lifespan.
  •  Interest and experience in working in community health setting, with nurse practitioners and as part of a multi-disciplinary team 
  • Experience with and sensitivity to issues impacting on clients and communities accessing the Centre 
  • Experience with immigrants and refugees, seniors, youth, individuals or families with low/no income, people living with mental illness and/or people who are homeless or at risk of being homeless 
  • Knowledge of the community health centre sector is an asset.
