Job Description
Trillium Health Partners (THP) and LOFT are partnering to launch a transitional care service program in the community to facilitate the transition for patients from hospital ALC bed units into the community through a short-term reintegration transitional care program.
The Primary Care Program is seeking two (2) Family Medicine Physicians for Courtesy positions to join the LOFT Team. The physicians will provide going Primary Care at the LOFT site located at 3705 Bathurst St, Toronto. The site is a 64 unit building that will be operated by LOFT. THP and LOFT will partner to transition clients from THP to LOFT’s program, and to support client needs at the program site. The successful candidates will be providing care for up to 24 patients under the LOFT/THP Partnership in a shared care model.
Candidates should have experience with all aspects of primary care and be able to provide the full spectrum of care, including dementia care, mental health, managing medical co-morbidities and chronic disease management. The successful candidate will be credentialed with THP but will be solely based out of LOFT site.
Compensation is an hourly stipend, plus FFS OHIP billing. There are no requirements for after hour care.
