📍 Location: 372 Hollandview Trail, Aurora, ON 🕒 Position: Full-Time Family Physician (1 or 2 Physicians) 👥 Patient Base: 1,500 patients ready for management
About Us
Aurora Medical Clinic is a well-established healthcare facility dedicated to providing comprehensive primary care services to the community. We are seeking one or two dedicated Family Physicians to join our team and take over an existing patient roster of 1,500 individuals.
To ensure a smooth transition, we offer a structured induction program to help the physician(s) settle in, familiarize themselves with the clinic's workflow, and integrate seamlessly into our healthcare team.
Key Responsibilities
-Provide comprehensive primary care to patients of all ages.
-Diagnose and treat acute and chronic illnesses.
-Conduct preventative care, including vaccinations and health screenings.
-Perform routine medical procedures and minor surgeries.
-Offer prenatal and postnatal care.
-Provide mental health support and chronic disease management.
-Collaborate with specialists and healthcare professionals for referrals and consultations.
-Maintain accurate electronic medical records (EMR).
-Participate in after-hours care, as required by the Family Health Organization (FHO) model.
Flexible Patient Management Options
-Single Physician Model: One physician takes over the full 1,500-patient roster.
-Shared Model: Two physicians split the existing patient list and enroll new patients through meet-and-greet appointments.
Qualifications & Licensing
-Medical Degree from an accredited institution.
-Completion of Family Medicine Residency Training.
-Certification by the College of Family Physicians of Canada (CFPC).
-Licensure with the College of Physicians and Surgeons of Ontario (CPSO).
-Eligibility for Ontario Health Insurance Plan (OHIP) billing.
Compensation & Remuneration
This position follows the Family Health Organization (FHO) payment model, which includes:
-Capitation payments based on enrolled patients.
-Fee-for-service billing for non-capitated services.
-Bonuses and premiums for preventative care and chronic disease management.
-Income stabilization for new physicians joining a FHO.
-EMR: Telus PSSuite, Ocean eReferral, Ocean web forms, Ocean secure messaging
