Position Statement/Summary

This position offers an opportunity to practice within one of Ontario's largest multi-service community health organizations and support clients with complex medical health challenges and social needs through a collaborative and integrated model of care.

As a member of the inter-professional primary health care team, you will provide comprehensive primary health care with a focus on health promotion to clients of all ages, on and off-site, including home visits; work independently and as part of an inter-professional team, and participate in on-call services. You will also be involved in organizational, team-based or individual quality and performance improvement projects and initiatives that improve client outcome and increase access to care.

Key Responsibilities

·      Provides ongoing client-centered, comprehensive primary medical care to clients of all ages, including assessment, diagnosis, treatment and follow-up, counselling, screening, referral, and client education.

·      Provides comprehensive primary medical care to populations facing multiple barriers, ensuring access to those most vulnerable.

·      Provide itinerant services working from different locations to create greater access to populations facing barriers to care.

·      Participate in the rotation for on-call among physicians, providing advice to patients (in-person, via phone or other virtual platforms), and dealing with urgent lab results.

·      Collaborates with other members of the inter-professional team; and consults as required with Nurse Practitioners to enable them to work with their full scope of practice.

·      Sees clients primarily on site – on both scheduled and urgent basis – and as needed in the community through outreach, off-site clinics, home and hospital visits.

·      Participates in primary health care programs and service delivery in partnership with other institutions and agencies as required.

·      Initiates, participates in, or leads appropriate in-service training sessions.

·      Ensures that infection control policies and procedures are followed.

·      Fosters clients’ self-help, mutual aid and capacity building by assisting with the development, implementation and evaluation of health promotion programs that address the needs of the community, as appropriate.

·      Where appropriate, develop partnerships with other agencies to improve health care service provision and better address the needs of the community.

·      Maintains complete and accurate client records, assessment notes and required correspondence.

·      Safeguards client records and assures confidentiality and privacy of client information.

·      Participates in the implementation of the organizational quality improvement initiatives and works towards achieving organizational goals to ensure access to primary health care services.

·      Contributes to programs and services cycle of planning and evaluation, and participates in center-wide planning, committees, and team meetings as appropriate.

·      Interest in participating in the training and education of new physicians (e.g., residents, medical students, and other trainees) and other health care professionals.

·      Participates in inter-professional research with colleagues - internal and external to the Centre, as appropriate.

·      The physician reports to the Manager, Interprofessional Primary Care and works collaboratively with medical leads.

·      Maintains all licenses, certificates, and standards related to the ability to practice as an MD in Ontario.

Qualifications

·      Degree in medicine from a recognized school or faculty of medicine.

·      Current license to practice medicine from the College of Physicians and Surgeons of Ontario.

·      Successful completion of a two-year post graduate residency program accredited by the College of Family Physicians of Canada and an active member of the College.

·      Experience providing comprehensive primary medical care to populations facing multiple barriers and with comorbidities, including mental health and addictions.

·      Strong clinical skills.

·      Demonstrated ability, commitment to and knowledge of community health care.

·      Demonstrated ability to work in a team-based model and within an inter-professional team environment.

·      Excellent interpersonal skills with clients in a culturally diverse practice population.

·      Excellent communication, decision-making, problem-solving, conflict management and time management skills.

·      Proficiency in the use of computers, Microsoft Office applications and electronic health records.

·      Second language, relevant to catchment area populations, is an asset.
