Key Responsibilities

• Provide comprehensive primary care services to a diverse patient population across all age groups.
• Perform thorough patient assessments to diagnose and manage acute and chronic medical conditions.
• Develop and implement individualized treatment and care plans.
• Provide ongoing management of chronic diseases and coordinate referrals to specialists when appropriate.
• Deliver preventive care services, including health screenings, immunizations, and patient education.
• Maintain thorough and accurate medical documentation in compliance with regulatory and professional standards (Experience with Accuro/Telus EMR System is an asset).
• Collaborate effectively with the interdisciplinary healthcare professionals (including pharmacists and specialists like Psychiatrists, Allergists, and Rheumatologists) to ensure optimal patient outcomes.

Qualifications & Skills

• CPSO Physician's license / CPSO Number (required).
• Experience practicing in hospital and/or community clinical settings.
• Strong understanding of primary care principles and best practices.
• Demonstrated compassion, professionalism, and strong interpersonal skills.
• Proficiency in medication management and clinical decision-making.
• Excellent communication and patient engagement abilities.
• Ability to work collaboratively within a team-based care model.
