FHO+ PHYSICIAN POSITION IN ESTABLISHED FHO/FHT 

An opportunity to join a well-established FHO/FHT in Oakville. We are looking for 1 physician that is interested in becoming part of the group practice.  

The practice operates with a total of 6 family physicians. The roster for the practice is around 7500 patients. One physician is ready to retire and seeking replacement for this FHO position. 

We provide comprehensive longitudinal care. Physicians work closely with the FHT allied health staff including nurse practitioners, nurses and a social worker. There is shared coverage of weekend after hours clinics and each physician work one evening per week. Our support staff are experienced and have been working with our clinic for many years. 

The physician group works closely with each other on all aspects of the FHO+ management, and it is expected that any new physicians should be able to work well with the team as a whole. 

Charting is through the Accuro EMR. AI scribes are actively used by providers. 

 QUALIFICATIONS 

Doctor of Medicine (MD) degree from accredited institution 

Specialty Certification in Family Medicine from the College of Family Physicians of Canada (CFPC)

Active License to practice from the College of Physicians and Surgeons of Ontario (CPSO)

Membership in the Ontario Medical Association (OMA) 

Insurance via Canadian Medical Protective Association (CMPA)
