Job Title: Physician - Senior Community Primary Care

Location: Markham, Ontario – On-Site at Two Partner Senior Communities


About Perennial

Perennial delivers integrated primary care and wellness services directly within retirement residences and senior communities, to meet older adults where they are. By proactively addressing the needs of seniors, we improve quality of life, reduce unnecessary hospital visits, and help ease pressure on the public healthcare system.

Today, Perennial operates in over 29 residences across Ontario, Saskatchewan, Alberta, Manitoba, and Quebec, with continued plans for growth and expansion.

Our interdisciplinary team includes physicians, nurse practitioners, registered nurses, physiotherapists, occupational therapists, dietitians, and other allied health professionals  working together with one clear mission: to help residents stay healthy, active, and supported within their communities.

Position Summary

Perennial is seeking a compassionate, collaborative, and community-minded physician to support two senior communities in Markham, Ontario with a combined population of approximately 2,500 residents.

This is an opportunity to build and grow a meaningful seniors-focused practice within an established community setting. The physician will have the opportunity to roster residents directly and bill OHIP accordingly, or alternatively provide services on a fee-for-service OHIP billing basis, depending on the physician’s practice model and preferences.

The role is expected to require approximately one on-site day per week out of our Perennial care doctor’s office, with flexibility in scheduling. You will work closely with the Perennial team and community staff to provide accessible, proactive care to residents, including ongoing primary care, chronic disease management, and urgent medical support as needed.

Key Responsibilities

  • Primary Care Delivery – Provide high-quality primary care and medical assessments to residents within the senior communities.
  • Resident Rostering / Practice Development – Build and manage a resident patient panel, including the opportunity to roster eligible residents where appropriate.
  • Continuity of Care – Support ongoing management of chronic conditions, medication reviews, preventive care, and follow-up planning.
  • Collaborative Care – Work closely with Perennial’s interdisciplinary team and community staff to coordinate care and improve access for residents.
  • Documentation & Orders – Maintain timely clinical documentation and complete required medical orders, prescriptions, requisitions, and referrals.
  • Family & Resident Communication – Communicate clearly and professionally with residents, families, substitute decision-makers, and care teams as appropriate.

Qualifications

  • Licensed for independent practice and in good standing with the College of Physicians and Surgeons of Ontario (CPSO)
  • Experience in family medicine, geriatrics, retirement home care, community medicine, or complex/chronic disease management preferred
  • Strong clinical judgment and comfort working in a community-based care setting
  • Comfortable practicing independently while collaborating with an interdisciplinary support team
  • Proficient with electronic medical records and general digital documentation workflows
  • Excellent communication skills and a warm, professional bedside manner
  • Ability to communicate in Mandarin and/or Cantonese is considered a strong asset, given the linguistic needs of portions of the resident population

Compensation & Schedule
