PERMANENT PART-TIME PHYSICIAN POSITION
Approximately 20 hours per week (600 patients)
Eastern Ottawa Community Family Health Team (EOCFHT)
The Eastern Ottawa Community Family Health Team (EOCFHT) provides primary health care services focused on prevention, health promotion, and community wellness. As a Francophone team offering services in both official languages, we value interdisciplinary collaboration, high-quality care, innovation, and a strong commitment to providing services in French.
Our team includes, among others: nurse practitioners, registered nurses, registered practical nurses, dietitians, a pharmacist, a diabetes education team, a mental health team, an administrative and a management team.
RESPONSIBILITIES
· Provide comprehensive primary health care to a diverse group of patients of all ages and their families, while considering the social determinants of health. This includes health assessments, diagnosis, screening, counselling, education, treatment, referrals, and consideration of psychological, social, and environmental risk factors.
· Act as a resource by providing expertise and support to the interdisciplinary team, including consultation services to Nurse Practitioners.
· Participate in the planning, development, and implementation of EOCFHT programs (health education programs, screening programs, themed workshops).
· Participate in interdisciplinary team meetings, committees, and activities that support the growth and development of the EOCFHT.
· Follow established practices to ensure medical records are accurate, complete, and up to date.
· Participate in the collection and analysis of data for administrative, planning, and evaluation purposes.
· Work with the Executive Director and other team members to develop protocols and procedures that improve the quality of services and patient care.
· Maintain and expand professional knowledge through continuing professional development recognized by the College of Physicians and Surgeons of Ontario.
· Participate in the education and supervision of medical and nursing students at the EOCFHT.
· Be available for on-call duties when required to meet operational needs and provide services in different work settings (for example, different clinic locations).
· Perform other related duties (for example, research projects, community representation activities, etc.).
ESSENTIAL QUALITIES
· Strong commitment to providing health care using a community health approach.
· Understanding of the needs and challenges of a diverse client population (culture, age, etc.) with multiple and complex health issues.
· Excellent communication, collaboration, teamwork, and relationship-building skills within an interdisciplinary team.
· Ability to adapt, solve problems, and manage conflict and stress.
· Strong time management and workload management skills.
· Working knowledge of Microsoft Word, Excel, and electronic medical records (EMR) PS Suite.
· Functional proficiency in both official languages (French and English), spoken and written.
QUALIFICATIONS AND EXPERIENCE
· Degree in Family Medicine from a recognized university.
· Certification from the College of Family Physicians of Canada.
· License to practice issued by the College of Physicians and Surgeons of Ontario.
· Experience practicing family medicine, preferably in a community health setting.
