Join Southlake’s New Extensivist Clinic – A Transformative Approach to Outpatient Care

Southlake Health is seeking a licensed General Internist to be part of a groundbreaking, patient-centered model of care in our newly launched Extensivist Clinic. This outpatient program, the first of its kind in the region, is designed to deliver innovative care that bridges the gap between hospital and community services.

About the Clinic

The Extensivist Clinic is a dynamic collaboration of interdisciplinary professionals, including Nurse Practitioners, RNs, RPNs, Social Workers, Pharmacists, and more—working together to provide integrated care that supports patients in avoiding unnecessary hospital admissions and enables earlier, safer discharges.

Located on Southlake’s hospital campus in Newmarket, Ontario, this clinic draws on the success of our Southlake@home program, a provincial leader in transitional care. The clinic is open Monday to Friday, with plans to expand to a 7-day model in future phases.

A physician stipend will be provided during the clinic’s initial growth period to support the successful integration and ramp-up of patient volumes.

Key Responsibilities

·        Assess and manage complex patients in an outpatient clinic setting

·        Deliver comprehensive internal medicine care, including stabilization and optimization

·        Prescribe and manage medications and treatments

·        Maintain accurate and timely documentation in the hospital EMR system

·        Collaborate with hospital, primary care, and community partners to meet health quality objectives

·        Provide leadership and direction to the care team, including care managers and allied health professionals

·        Participate in quality improvement initiatives and other duties as assigned

Required Qualifications

·        Medical Doctor (MD) designation

·        Certification in Internal Medicine

·        Eligible to practice general internal medicine in Ontario

Preferred Qualifications

·        Experience in value-based healthcare models

·        Demonstrated success working in collaborative, interdisciplinary teams

·        Background in outpatient or transitional care settings
