Job Description

PROGRAM:  Stronach Regional Cancer Program

Department:  Member of Division of Medical Oncology/Haematology - Within Dept. of Medicine

POSITION:  Full Time General Practitioner in Oncology Inpatient (Occasional evenings and on call responsibilities)   


PROGRAM OVERVIEW:

The Stronach Regional Cancer Centre (SRCC) is a regional hub for delivery of services and programs for patients and families living in the York Region, North York and South Simcoe County. SRCC is the largest by population in the province and consistently ranks as one of the top Regional Cancer Programs based on provincial performance indicators. SRCC provides a full range of oncology services including diagnosis, treatment, and follow-up within the modalities of medical oncology, radiation oncology, as well as palliative care and end of life care. SRCC is the only Cancer Centre in the Central Region to provide Radiation Oncology. 


POSITION OVERVIEW:

The Clinical Associate – General Practitioner in Oncology (GPO) as an important member of the Stronach Regional Cancer Program team provides direct patient care to patients and families admitted to the Cancer Program on the inpatient Cancer Unit (CAN) in collaboration with the Oncologists and other team members including Nurse Practitioner support. 

Primary responsibility will be the oncology inpatient population and to directly manage inpatients admitted to the Medical Oncology/Malignant Hematology service, from initial contact to completion of discharge paperwork, notes and referrals for outpatient services. 

The in-patient service consists of 22 admitted patients with a variety of oncologic needs including active chemotherapy complications as well as symptom management and palliative care. The inpatient CAN unit provides a dedicated open bed to admit requiring the need for optimal/rapid assessments to offload and maintain patient access and bed flow. The GPO will assist with optimal bed flow to the unit and facilitate efficient patient discharge. 


 DUTIES AND RESPONSIBILITIES:

1. Completing initial Medical Oncology/Malignant Hematology in-patient and emergency department consultations with appropriate and immediate sub-specialty medical oncology supervision/ back-up available. 

2. Complete relevant oncology diagnostic and therapeutic procedures (i.e. paracentesis, thoracentesis, bone marrow biopsies and lumbar punctures).

3. Under the supervision of a Medical Oncologist, assume the MRP role for Oncology patients for the 22 inpatient beds on the Cancer unit. 

4. Providing the point of contact and resource for the in-patient inter-professional team regarding goals of care and discharge planning for admitted Medical Oncology/Malignant Hematology patients.  

5. Liaising with other hospital specialties to obtain consultation and support for admitted Medical Oncology/Malignant Hematology patients where required. 

6. Liaise with SRCC Physicians and Nurse Practitioners for smooth transitions to and from the Outpatient Cancer Clinics.

7. Engaging with the other physicians’ extenders for radiation oncology and hematology to ensure appropriate care of shared malignant hematology and medical oncology patients

8. Implement treatment plans and follow-up for patients requiring routine systemic therapy or are under a research program in the community cancer clinic.

9. As part of the agreed upon time commitment, act as a liaison with community physicians to ensure continuity for patients under their care. 

10. Work with the Cancer Care Team to promote the patient–centered model of care and participate in interdisciplinary rounds, educational events, multidisciplinary case conferences as well as patient/family conferences where appropriate.

11. Providing educational support to learners and the inter-professional team on the unit. 

12. Engage in discussions around optimization of the General Practitioner in Oncology resource, as it may pertain to ongoing education, roles and responsibilities of the GPO as well as growth and development of the GPO role within the Oncology Program.

13. As part of the agreed upon time commitment, participate in administrative committees such as Cancer Care Program and others as required.

Qualifications:

1. Member in good standing with the College of Family Physicians of Canada (CFPC) and/or general internal medicine with the College of Physicians and Surgeons of Ontario.

2. Licensed to practice medicine in Ontario.  

3. Eligible for appointment to Southlake Health Medical staff.

4. Minimum of two years’ experience in an acute care hospital with experience in cancer will be given strong preference.

5. Knowledge of clinical, administrative, and system-wide issues with respect to cancer.  

6. Demonstrated interest in working with cancer patients and families.

7. Demonstrated ability to work both independently and as a member of an interdisciplinary team.

8. Demonstrated ability to maintain positive relationships with other health care team professionals and able to contribute to the development of an environment conductive to excellent patient care.

9. Responsible, energetic and resilient individual with an above average ability to prioritize and organize activity in a busy environment.  

10. Excellent interpersonal skills and ability to build effective relationships with clinical and administrative stakeholders.

11. A strong professional presence and ability to present to large and small audiences in a concise, articulate and dynamic fashion.
