Disera Medical Centre, an established multi-specialty family practice and walk-in clinic in Thornhill, is seeking a Geriatrician to join our growing team. We support a large and diverse patient base across a team of experienced family physicians, specialists, MOAs, and administrative staff.

Our clinic serves a significant aging population in the Thornhill, Richmond Hill, and North York corridor, and we are looking to offer dedicated geriatric care in-house to reduce wait times and improve outcomes for our patients.

Responsibilities:

• Provide comprehensive geriatric assessments and ongoing management for elderly patients

• Diagnose and manage complex multi-morbidity, cognitive decline, dementia, and frailty

• Conduct falls risk assessments, medication reviews, and polypharmacy optimization

• Collaborate with family physicians on shared care plans for complex elderly patients

• Provide consultation and follow-up for patients referred from within the clinic and externally

• Complete documentation, referral correspondence, and reporting in EMR

• Participate in multidisciplinary care coordination as appropriate

Qualifications:

• MD with current CPSO licensure

• FRCPC in Geriatric Medicine

• Valid CMPA coverage

• Strong clinical, communication, and interpersonal skills

• Experience managing complex elderly patients in a community setting preferred
