Disera Medical Centre, an established multi-specialty family practice and walk-in clinic in Thornhill, is seeking an Internal Medicine Specialist to join our growing team. We support a large and diverse patient base across a team of experienced family physicians, specialists, MOAs, and administrative staff.

Complex chronic disease management is one of our highest referral areas, and we are looking to bring internal medicine services in-house to support our family physicians with patients who need a higher level of diagnostic and management expertise.

Responsibilities:

• Provide specialist consultations for complex medical patients referred from family practice

• Diagnose and manage multi-system conditions including diabetes, hypertension, heart failure, COPD, and complex multi-morbidity

• Conduct comprehensive medical assessments and develop management plans

• Collaborate with family physicians on shared care for complex patients

• Interpret and act on laboratory, imaging, and diagnostic results

• Complete documentation, referral correspondence, and reporting in EMR

Qualifications:

• MD with current CPSO licensure

• FRCPC in Internal Medicine

• Valid CMPA coverage

• Strong clinical, communication, and interpersonal skills

• Experience in a community or outpatient setting preferred
