Disera Medical Centre, an established multi-specialty family practice and walk-in clinic in Thornhill, is seeking a Nephrologist to join our growing team. We support a large and diverse patient base across a team of experienced family physicians, specialists, MOAs, and administrative staff.

With a growing number of patients managing chronic kidney disease, hypertension, and diabetes-related renal complications, we are looking to offer nephrology consultations in-house.

Responsibilities:

• Provide specialist consultations and ongoing management for patients with kidney disorders

• Diagnose and manage chronic kidney disease, glomerulonephritis, electrolyte disorders, and hypertension-related renal conditions

• Monitor renal function, adjust treatment plans, and coordinate pre-dialysis care

• Collaborate with family physicians on shared care plans for patients with renal comorbidities

• Interpret and act on laboratory and diagnostic results

• Complete documentation, referral correspondence, and reporting in EMR

Qualifications:

• MD with current CPSO licensure

• FRCPC in Nephrology

• Valid CMPA coverage

• Strong clinical, communication, and interpersonal skills

• Experience in a community or outpatient nephrology setting preferred
