Job posting: Part-time Physician for home based palliative care (September 2025) in London, Ontario
The London Home Based Palliative Care (LHPC) Team is inviting applications for an experienced clinician (internist) to join their community based palliative care team.
Dr. Hamilton is a FRCPC palliative medicine specialist and the lead physician for the LHPC team. Our team is the specialist home palliative care team in London. We are seeking a physician that demonstrates a passion and proven skillful management with managing home based palliative care patients with cancer and non-cancer illnesses. The successful candidate will require at least 3- 5 years of clinical experience along with training in managing complex medical/palliative patients. Our LHPC Team is committed to teaching our future leaders in palliative care and a preference will be given to candidates with a similar desire for an active involvement with learners. There is full back up from palliative care physicians who are available for support while working on this team.
The core responsibilities for this job will include:
- Commit to providing 5-8 home visits, two or three days per week (more days per month may also be available)
- One weekend of home on-call every 4 weeks (additional weekends may be available)
- Available to cover the LHPC team 4-6 full weeks (Monday to Friday)
- 4-8 weeks of allocated personal time off the LHPC team service per year
Remuneration:
- 70/30 fee split for all OHIP billings, incentives and the on call stipend during the first 3 months
- After 3 months, if the physician is meeting the needs of the LHPC team patients, the fee split will decrease to 80/20
- Estimate minimum income over $400,000.00 per year (after fee split) based on 8 home visit per day, 3 days per week, 13 weekends per year of on call and 4 full time weeks of coverage and 4 weeks of personal time off. The income would significantly increase as the amount of work were increased.
Other details:
- Full access to EMR (no charge for this service)
- Call stipend is available
- Home visits will mostly be arranged for the doctor during the weekdays and for their on call weekends
- On-Call responsibilities are not an onerous aspect of this job. There are very few urgent pages received while on-call and only the very rare page during the night. Our team physicians almost never need to attend home visits after dark.
